He was predeceased by: his mother Janet C. Puchalski (Davies). He is survived by: his father Martin F. Puchalski (Janet) of Mt. Carmel; his daughter Kasey Puchalski of Shamokin; his siblings, Kim Berezovske (Dirk) of Blue Bell, Jackie Dancho (Dave) of Numidia and Derek Puchalski (Kelly) of Springfield; his nephews, David Dancho, Jared Dancho, Dirk Berezovske Jr., Justin Dancho, Derek Puchalski Jr. and Ethan Puchalski; his niece Kristen Karanikas (George); and his pet Nutso. He is also survived by many aunts, uncles, cousins.
