He is survived by: his father Joseph Chulvick; his significant other Jacqueline Rupp-Finegan of Moosic, PA; his children, Tyler Chulvick and Kaley Chulvick of Dunmore, PA; and his sister MaryJo Chulvick (Frank Foft) of Columbus.
