He was predeceased by: his parents, Frank Belak and Sophie Belak; and his son Jeff. He is survived by: his wife Kathleen Granahan; his grandchildren, Jacob Bertha of Lake Mary, Fla. and Callie Bertha (Zach Beeman); and his brothers, Ray (Barbara), Henderson and Nev. He is also survived by nieces, nephews, and cousins.
In lieu of flowers, memorial contributions may be made in Frank's memory to the ALS Association or the donor's favorite charity.
