He was predeceased by: his parents, William H. Cook and Mary Jane Cook (McCloskey); and his niece Elizabeth Cali. He is survived by: his close friend Frances Knott (Markel); his siblings, Gary A. Cook (Patty) and Ruth E. Smith (Richard); his niece Kelly Baker (Scott); his nephew Trevor Smith (Cristina); his great-nieces and great-nephews, Brody, Trevor, Peyton, Lucas, Cameron, Avery and Julia; and also, Kathy Knott Asche (Mike), Derek, Lauren, Olivia, Kurt Knott (Abby), Vivian, Quinton and Lydia.
