He is survived by: his daughter Alexis Forte-Wadsworth; his son-in-law Danny Wadsworth of California, PA; his daughters, Brooke Forte and Brandee Forte of Bentleyville; his grandchildren, Ava, Jayden and Carmella Wadsworth; his brother Jeffrey Forte (Kathleen) of Washington Township; and his nieces and nephews, Brandon, Caitlyn and Autumn Forte.
