He was predeceased by: his parents, Donald Hack and Mary Hack (Nause); his brother Daniel Hack; his sister-in-law Bonnie Hack; and his uncle Harold Hack.
He is survived by: his wife Charlotte M. Hack; his children, David Hack, Jr. of Millville and Stacy Hack (Justin Enterline) of Bloomsburg; his granddaughter Kylenna Hack; his aunt Jackie Hack of Millville; his niece Melissa Hack of Berwick; his nephew Danny Hack, Jr. of Mifflinville; his cousins, Nevin Hack of Millville and Barb (Gary Crawford) of Millville; and his half-brother Damon Hack of Bloomsburg.
