She was predeceased by: her parents, Louis Mainolfi and Antoinette Mainolfi (Mazza); her husband Theodore "Ted" Hazlett; and her brother Anthony Mainolfi. She is survived by: her sons, Jim Hazlett (Rita) and Ted Hazlett (Amy); her grandchildren, Elizabeth Hazlett, James Hazlett, Jr., Vanessa Prothe (Dan) and Charisse Losecar (Joe); her great grandchildren, Theo and Freddie Prothe; and her siblings, Josephine "Josie" Puglesse (late Dave), Mary Lou Merlino (late Sonny), Lena Weber (Lou), Barbara "Babba" Puglesse (Ap) and James Mainolfi.
