He was predeceased by: his parents, Ralph Hazlett and Ruth Hazlett (Button); and his sisters, Barb and Bonnie. He is survived by: his sons, Edward Hazlett, Eugene Hazlett (Amy), Shawn Hazlett (Nicole) of Allentown, PA and Stephen Hazlett (Kimberly) of Wellsboro, PA. He is also survived by seven grandchildren and two great grandsons.
