She is survived by: her son David A. Kozak (Marianne); her grandchildren, Kim Gouker (Pat), Kris Gates (Adam) and David Kozak, Jr. (Stacey); her great grandchildren, Patrick Gouker II and Eva Gouker; her niece Dianne Poff (Culbert); and her great-nephew Christopher Michael Poff.
