He is survived by: his mother Shirley Phelps (Thellman) of Meadville; his spouse Kasandra Foltz; his cousin Tyler Phelps; his children, Hunter Phelps and Rion Foltz; his grandmother Marian L. Phelps of Meadville; his sisters, Melissa Woodworth (David Cogswell) of Sandy Lake, Debbie Nichols (Greg) of Spencer, WV and Andrea Brodeur of Topsham, ME; his parents-in-law, Syleste Foltz of Meadville and Howard Foltz of Centerville; and also, Cynthia Phelps, Debbie Lowman (Dan), Martha Phelps (Bill Halstead), Kelly Penn, Tyler Phelps, Ryan Phelps (Amanda), Jordan Woodworth (Kayla Lewis) and Deven Woodworth (Carli).
