She was predeceased by: her spouse Edward J. Caputo; and her siblings, Dolores Fry and Helen Rosen. She is survived by: her children, Edward F. Caputo (Sherri), Tina Caputo-Griffel and John J. Caputo; her grandchildren, Gina M. Mahfood, Joseph A. Griffel, John A. Griffel, Jennifer L. Rouse, Edward J. Caputo II, Michael J. Caputo (Nicole) and Marc E. Caputo; her sibling Margaret Gattrell; and her siblings-in-law, Harry Fry and Theresa Caputo. She is also survived by nieces, nephews, cousins and friends.
The family suggests donations in Hazel's memory to the American Diabetes Association.
