She was predeceased by: her parents, Albert A. Fazek and Ruth Fazek (Bates); and her brother Mark Fazek. She is survived by: her husband Matthew Shaffer; her son Joshua Shaffer; her granddaughter Leilynn Shaffer; and her brother Neil Fazek.
