He was predeceased by: his parents, Antonio Azevedo Jr. and Mila June Hall; his foster parents, Elmwood Grubbs and Elaine Grubbs; and his son Phillip Azevedo.
He is survived by: his wife Patricia A. Azevedo (Hansen); his daughter Venus M. Laboda (Jeffery); and his grandsons, Matthew Koenig and Dominic Anthony Azevedo.
