She was predeceased by: her parents, Andrew Zajdel and Honorata Zajdel (Godzisz); her children, Jeanie DeLuca and Larry; and her siblings, John, Mary, Felix, Frances, Anne, Steve, Dorothy and William.
She is survived by: her husband John Kozak; her sons, Bill (Debbie) and Jim Kozak (Mary); her children-in-law, Dominic DeLuca and Teresa; and her great grandchildren, Carly, Maddie and Aly.
