She was predeceased by: her parents, Frank Carolus and Catherine Carolus (Harris); and her son Scott Dorman. She is survived by: her husband John Dorman Sr.; her children, John Dorman Jr. (Teresa) of Duncannon, Elaine Kearns (Dorman) of Lewistown and Amy Dorman (Randall Pollock) of Lewistown; her grandchildren, JohnPaul, Timothy Nelson, Lisa Elaine Hopple (Kearns) and Ashley Marie Francis (Dorman); her great grandchildren, Elayna, Caleb, Gavin Hopple, Lexi, Jacob, Malachi Kearns, Evangeline and Victoria Francis; and her brother Harold "Jake" Carolus (Donna) of McVeytown.

In lieu of flowers, memorial contributions in Hazel's name may be made to the American Heart Association, 1019 Mumma Road, Wormleysburg, PA 17043.
