He was predeceased by: his parents, John Cizek and Anna Cizek (Sykora); his wife Ellen Metje; and his siblings, Vernon, Norman Cizek and Helen Wagner (Madeline Renard).
He is survived by: his children, Randy R. Cizek of Covington, Laura E. (Thomas Radawiec) and Sally A. (Joe Burget); his grandchildren, Zachary Cizek, Andrew and Grace Burget; his great grandchild Rayne Cizek; and his close friends, Barbara Goss and Linda Hoffer. He is also survived by numerous nieces and nephews.
