He was predeceased by: his parents, Jesse Cook and Evelyn Cook (Nemitz); and his sister Nancy Grace. He is survived by: his children, Brian Cook of Hayward California and Renee Cook Booher of Jacksonville Florida; his grandchildren, Daryl Cook (Lauren), Sean Cook, Matt Booher (Laura) and Melinda Booher Hays (Robert); and his great grandchildren, Kaden, Alyssa, Tyler, Abigail, Nathaniel, Caleb and Joshua.
