He was predeceased by: his parents, Edward A. "Whitey" Slezak and Catherine Slezak (Yancy); and his wife Joann E. Slezak (Pennesi). He is survived by: his children, Anthony Slezak (Cheryl) of Latrobe, Donna Calabrese (Anthony) of Export and Diane Barchesky (Edward) of Derry; his grandchildren, Craig Calabrese, Lisa Krall, Amy Butenko, Beth Addis, Lynn Simms, Sarah Slezak, Julie McLuckie and Mike Slezak; his great grandchildren, Abigayle Meier, Brian Krall, Jason Krall, Emma Simms, Hunter Simms, Carrie Addis, Brooke Addis, Joshua Butenko, Luke Butenko, Kelly Butenko and Connor McLuckie; and his sisters, Mildred Hlad of Latrobe and Irene Bisi (Ronald) of Latrobe. He is also survived by numerous nieces and nephews.
