He is survived by: his wife Shirley A. Horn (Smith); his children, Cris Chambers (Jack Chambers) and Gerry Horn (Lisa Horn); his grandchildren, Brittany Bormann (Bradford Bormann) and Jared Horn; and his great grandchild Theo Bormann.
