He was predeceased by: his parent Howard W. Lewis; and his siblings, Paul Lewis, Howard L. Lewis, Steve D. Lewis, John Lewis, Pauline Kapp and Ruth Confer.
He is survived by: his parent Mae Bashline Lewis; and his siblings, Dean Lewis (Althea) of Fern and Winnie Mae Hook of Knox. He is also survived by numerous nieces and nephews.
