She was predeceased by: her husband Robert K. "Bob" Jack; and her siblings, Gordon, Bob, Rodger, Richard Olinger and Joyce Cornman.
She is survived by: her children, Larry Jack (Nancy) of Kittanning, Debbie Jack of Kittanning, Missy Hobaugh of Kittanning, David Jack (Cindy Boyer) of Kittanning and Bob Jack of Kittanning; her grandchildren, Travis Jack (Morgan) of Kittanning and Kimberly Jack (Dillon Schaub) of Kittanning; and her great grandchildren, Easton, Evelyn Jack, Jordan, Logan Dingess and Cody Schaub.
