He was predeceased by: his parents, Howard Jack and Lois Jean Jack (Hileman). He is survived by: his wife Judy Jack (Rafetto); his children, Aaron M. Jack (Allison) of Cleveland, OH, Angela M. Bedford-Jack (Emma) of Pittsburgh and Abigail M. Maley (Blayne) of Cincinnati, OH; his grandchildren, Grayson Jack, Silas Bedford-Jack, Eleanor Bedford-Jack and Amelia Peters; and his siblings, Howard Jack (Paula) of East Brady, Barb Smith (Frank) of Ford City and Faith Jack of Gibsonia.
