He was predeceased by: his father Jim H. Fields, Sr.. He is survived by: his wife Jennifer; his children, Ryan and Kelly; his mother Sara; his brother John of Port Richie, FL; and his nieces, Jennifer, Hayley, Stephanie and Michelle.

In lieu of flowers, donations can be made to the American Cancer Society, American Red Cross or the Cholangiocarcinoma Foundation.
