He is survived by: his mother Jolene Phillips; his fiancée Kathy Carlson; his daughter Melissa Calzone (Marc) of Lakewood, NY; his grandchildren, Xander, Xavier Calzone and Zoey Calzone; his stepdaughters, Elisha Osmer (Brian) of Kane and Erin Baldwin (Adam) of Raleigh, NC; and his siblings, Jeff Crutchfield of Kane, Terry Crutchfield of Akron, OH, Brad Ransom and Tonia Langsdale of Euclid, OH. He is also survived by several nieces and nephews.
