He was predeceased by: his parents, Samuel Ernest and Ethel May Jacoby (Berkebile); his former wife Roberta (Snyder); his sisters, Lois and Helen; and his grandson Geoffrey.
He is survived by: his wife Linda Burkey; his children, Sherry Roxanne (Frank Blair) and Kevin Wayne Jacoby; his siblings, Sam and Rae; his grandchildren, Yvonne (Brian Krise), Roger Logue and Gregory Jacoby; and his great grandchildren, Katelyn (Devon Marshall), Abbigail, Daelan and Alexandria. He is also survived by three great great grandchildren.
In lieu of flowers, donations may be made to the family for assistance with funeral expenses.
