She was predeceased by: her parents, John Hebda and Mary A. Hebda (Olszewski); her husband Frank J. Slezak; her daughter Victoria Marie Slezak; and her siblings, Helen Tomera, Nellie Hyland, Genevieve Wallet, Anna Drzal, John, Henry, Ted and Joseph Hebda.

She is survived by: her children, Frank J. Slezak, Diane (Kiser) and Edward M. Slezak; her grandchildren, Matthew and Amanda Slezak; and her sister Mary Jane Kozak.

Family would like memorial contributions to St. Andrew Church.
