With heavy hearts, we announce the death of Mary Jane Kozak of Jerome, Pennsylvania, who passed away on May 11, 2021 at the age of 83. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, John Hebda and Mary A. Hebda (Oleszewska); her husband Kenneth G. Kozak; her siblings, Helen Tomera, Nellie Hyland, Genevieve Wallet, Anna Drzal, Florence Slezak, John, Henry, Ted and Joseph Hebda; and her great grandchild Nathan Neathamer.
She is survived by: her children, Kenneth D. Kozak (Tereska), Ronald P. Kozak (Cathy) and Karin (Galen Beam); her grandchildren, Alishia, Ashlee, Colleen (Joe), Ewelina (Mike), Gabriel and Sara; and her great grandchildren, Cheyanne, Azaria, Zania, Aria, Nona Bell and Jayden.
