He was predeceased by: his parent Dwayne Jacobs; his grandparents, Theodore R. Cook Sr., Harvey T. Berger Sr. and Olester Jacobs; his aunt Sylvia Thompson; and his uncle Randall Cook.
He is survived by: his parent Deborah L. Cook of Jeannette; his grandmother Dolores Berger (Cook); his children, Amiera Cook and Alexandra Williams of Greensburg; his siblings, Quindelyn Cook-Henderson, Dwayne Jacobs of Atlanta, Harvey Jacobs of Atlanta and Jermaine Jacobs of Atlanta; his aunt Rebecca Cook of Washington; his uncles, Theodore Cook of Jeannette and Jeffery Berger of Greensburg; and also, Sasha Garland of Jeannette and Kayla Cook of Myrtle Beach, S. C.. He is also survived by several aunts, uncles, cousins, nieces and nephews.
