He was predeceased by: his grandparents, Theodore, Lucy Antoniak and Thomas Lazarchik Sr.; his cousins, Kyle Lazarchik and Donald Antoniak; and his godmother Edna Streussnig.
He is survived by: his parents, Thomas Lazarchik Jr. and Suzon Laskey; his sister Alexandria Lazarchik; his stepfather Richard Laskey; and his grandmother Antonia Lazarchik. He is also survived by several aunts, uncles, and cousins.
