He was predeceased by: his parents, Homer B. Churchfield and Catherine Churchfield (Gryzbowski); his wife Elaine L. Churchfield (Maloney); and his siblings, Nelson Churchfield, Willliam Churchfield and Arline Volpe.
He is survived by: his children, Diane McDermott (late Mac) of Forest Hills, John D. Churchfield Jr. (Roni Jo) of Hempfield Township and Nancy Churchfield of Penn Township; his grandchildren, Adam McDermott, Meagan Worth (Josh), Mollie Minich (John), William Churchfield (Michael), Jacklynn Churchfield (Josh Humberger), Kylee Louies (Peter), Karly Reichert (Rick) and Austin Gnesda (Hannah Corey); his great grandchildren, Josh, Daphne, Connor, Kohen, Lila, Jackson, Lainey, Jett, Averie, Russell and Matthew; and his sister Naomi Zywiec of Wilmerding. He is also survived by nieces and nephews.
