He was loved and cherished by many people including: his parents, Lisa Moffatt and Lee Moffatt; his siblings, Zachary, Zaria and Gemma Moffatt; and his grandparents, Virginia, Michael Moritz, Patricia and Jere Moffatt.

In lieu of flowers, memorial donations can be made to The Western Pennsylvania School for Blind Children in honor of Jakob Moffatt.
