He was predeceased by: his parents, John Samuel and Olive Margaret Rader Perkins; his siblings, Edwin Harold (Nancy) and Evelyn Rae Odell (Rodney); and his nephew Matthew Wayne Perkins.

He is survived by: his wife Barbara Ferrari Perkins; his sons, Joe (Linda) and Chris Perkins (Kristin); his brothers, Robert Lee Perkins (Reba) and Jack Wayne Perkins (Claren); his grandchildren, Michael (Ashleigh), Nathan, Matthew, Courtney and Jake Perkins; and his great grandchild Ellie.

In lieu of flowers, contributions may be made to the American Cancer Society.
