He was predeceased by: his wife Mary. He is survived by: his children, Daniel S. Sulkowski (Amy Gilchrist), Elaine Vattimo (Brian) and Donald Sulkowski (Julie); his grandchildren, Shane, Karley, Lauren and Benjamin; and his great grandchildren, Kenny and Kyrie.

In lieu of flowers the family asks for a donation to St. Jude's Children's Hospital in honor of Dan's love of children.
