He is survived by: his parents, Michael Mayhew (Lori) of Clearfield and Valeria Walter (Mike) of Middleburg; his daughters, Kayleigh Mayhew, Olivia Mayhew and Emily Mayhew; his siblings, Amanda Mayhew, Jamie Brown, Shilo Walter and Tyler Quinn; his grandmother Mary Fulesday; and his former wife Samantha Mayhew. He is also survived by numerous aunts, uncles, nieces and nephews; and two great nieces.
