He was predeceased by: his parents, John W. Cook and Freda G. Cook (Stoner). He is survived by: his wife Trudy K. (Reed); his children, Cassi Cook-Ramper (Ronald) of Broad Top, Courtney W. Cook (Kristy) of Huntingdon and Travis J. Cook (Meshia Erb) of Huntingdon; his grandchildren, Caleb D., Colton W. Cook, Renlei K. Ramper, Cason W. Cook, Kaitlyn E., Madison A. Lemin, Tyson J., Trenton J. and Titan J. Cook; and his siblings, Sandy Curfman (Fred), Penny Weko (Mike) and Denny Cook.
