She is survived by: her children, Raymond, Perry Rotolo, Donna Smith (Howard) and Lisa Fedorka (Paul); and her siblings, Thomas Kozak (Bonjade), Richard Kozak (Sandy), Robert Kozak (Patricia), Leonard Kozak and Patricia Watters (Thomas). She is also survived by three grandchildren, three great-grandchildren.
