He was predeceased by: his parents, Jack Telford, Sr. and Betty Jane Shaffer (Miller). He is survived by: his wife Donna Webb; his children, Tina (Robert Kline), Lori (Ric Hiner), Timothy Shaffer, David Rogers, Amanda (Kent Ansell), Jack (Cassidy Shaffer III), James Dean Shaffer and Nichole (Travis Beal); and his siblings, Wilma Horner, Betty, Del Horner, Linda Hoffecker, Jim Shaffer, Sr., Tom Shaffer, Sr., Joe, Dar Shaffer, Pearl Shaffer, Mary, Kenny Beers and Tammy Williams. He is also survived by 14 grandchildren and 2 great grandchildren.
