He was predeceased by: his parents, Charles Samuel and Ethel Ruthann Fisher Cook; and his brothers, Merle and Roy Cook. He is survived by: his wife Barbara Ann Birckbichler Cook; his children, Beverly McBride (Rick) of Clintonville, Donald Cook (Ethel) of Hilliards, Mark Cook (Jordan) of West Virginia and Robert Cook (Renee) of Grove City; his grandchildren, Jeremiah, Sammy, Heather Cook, Amanda Summers, Hillary Hubbell and Frank Suhoney; and his brother Deryk Cook (Erma) of Hilliards. He is also survived by seventeen great-grandchildren.
In lieu of flowers, the family suggests memorials to St. Louis Catholic Church in West Sunbury, Pa. 16061.
