He was predeceased by: his parents, William Churlik and Doris Churlik (Cooper). He is survived by: his wife Sandra Churlik (Derrenbacher); his siblings, Doris Gatewood (Mark) of Pulaski and Wayne Churlik (Paula) of Fort Worth; and his nieces and nephews, Conor Churlik, Paul Churlik, Corey, Kevin, Gary Bender, Meredith Churlik and Cheryl Schwier.
