He was predeceased by: his parents, William Kozak and Mary Kozak (Zamborsky); and his spouse Shirley J. Kozak (Sears). He is survived by: his children, Nina Kozak, Stewart Kozak, Dean Kozak and Dawn Baughman; and his grandchildren, Denise, Daniel, Amanda, Emily, Hannah, Jessica, Megan and Ryan. He is also survived by eleven great-grandchildren; and three siblings.
