He was predeceased by: his parents, John Louis Shultz "Louie" and Rosella Shultz (Resh); his sisters, Sue Bowser and Sally Baumgardner; and his brother-in-law Gene Bowser.

He is survived by: his wife Marilyn (Muntzer); his daughters, Jennifer Chaffier (Shultz) and Jessica Shultz; his sons-in-law, Tim Chaffier and Tim Wynarczuk; his grandchildren, Lucianna, Dylan and Jackson Chaffier; his nieces and nephews, Kris Rudisill, Lauren Troop, Marcey Zeroll, Stacey Baumgardner, James O'Toole and Ed Baumgardner; and his siblings-in-law, Marsha O'Toole, Bill O'Toole and Ed Baumgardner.

To honor Jack's lifelong commitment to community service, contributions may be made in his memory to St. Matthew Lutheran Church; 30 W. Chestnut St.; Hanover, PA 17331 or to the Hanover Lions; ATTN: Treasurer, Robert Gotwalt; 52 Knisley Dr.; Hanover, PA 17331.
