He was predeceased by: his parents, Richard L. Lewis, Sr. and Laura Pulz (Payson); and his siblings, Linda Groover (Elmer), Debora McCormick (James) and Frank Lewis (Tammy).
He is survived by: his wife Mary Ann; his brother Ronald Lewis; and his children, Kimberly Crews, Amanda Kinzler, Erin DeTar (Eric), Nicole Thomas (Justin), Samantha Lewis and Sean Lewis. He is also survived by many grandchildren and great grandchildren and extended family.
In lieu of flowers the family has requested donations be made to your favorite charity or local little league group.
