He was predeceased by: his parents, John Hlebik and Magdalena Hlebik of Allison; and his siblings, Sally Burkhardt, Mary Faiella, Anna Rothman, John Hlebik Jr. and Andrew Hlebik.
He is survived by: his wife Shirley King Hlebik; his children, Michael E. Hlebik Jr. of Lake City, Tracy Hlebik Carlo of Lake Wales and Loreen Hlebik of Waltersburg; his sister Margaret Mastriano of New Jersey; his grandchildren, Jacob Wible, Kane Wible, Holly, Brianna and Emily Carlo; and his great grandchild Alaya Renae Carlo. He is also survived by several nieces and nephews.
