He was predeceased by: his parents, Wilbur Dale and Florence Polen (Houston); his wife Cathy Eileen Polen (Taylor); and his brother Robert Dale Polen. He is survived by: his sons, Jeremy Polen of Graysville and Nathan Polen of Washington; and his grandchildren, Lucy of Washington and Asa Polen of Washington.
