He was predeceased by: his parents, Peter Kostyzak and Ruth Kostyzak (Gross); his sons, Richard Kostyzak and William Kostyzak, Jr.; and his brother Richard Kostyzak.

He is survived by: his wife Enise M. Kostyzak (Donofrio); his son Michael R. Kostyzak of Glassport; his sister Ruth Ann Kostyzak of McMurray; his grandchildren, Billy, Brad, Brianna, Richie and Dania Kostyzak; and his great grandchildren, Richie, Ava and Anya. He is also survived by nieces and nephews.
