She was predeceased by: her parents, George Hlivko and Anna Hlivko (Tsapik). She is survived by: her husband James H. Cook; her siblings, Helen Kramer, Ann Yazembiak and Joseph Hlivko; and her children, JulieAnn Cook Davis (Ken), James Howard Jr. Cook (Melissa), George Joseph Cook (Sherry), Bettyanna Cook Young (Edmund) and Helen Marie Cook Smith. She is also survived by twelve grandchildren; sixteen great-grandchildren and many nieces and nephews.
