She was predeceased by: her parents, Vera Shaffer (Foreman) and James W. Shaffer; her granddaughter Taylor Smith; and her son-in-law Duane Fabrick. She is survived by: her husband John Folk (Jack); her children, Toni C. Fabrick and Michael A. Folk (Karen); her granddaughters, Christine Smith, Allicia Ruiz (Tony), Kelsey Smith, Hailey Fabrick, Lauren (Nick Huff) and Emily Folk; her great grandchildren, Vera Yvonne Ruiz and Wesley Scott Huff; and her brothers, James Shaffer (Pat), Ray Shaffer (Sharon) and Bob Hackett (Sally).
