He is survived by: his wife Leona L. Hazlett (Fosnaught); his children, Richard L. Hazlett Jr. of Harmar Township, Mark W. Hazlett (Ann) of Monroeville and Tammy L. Hazlett of Harmar Township; his grandchildren, Rob, Angeleah, Kayla, Matt and Mike; and his brother Rev. William Hazlett (Vernice) of Beaver. He is also survived by seven great grandchildren.
