He was predeceased by: his parents, Michael Kubiak and Rose Kubiak (Sawicki); and his siblings, Dolores Baldwin, Bernard, Joseph, Edward, Gerald, Leonard and Chester Kubiak.
He is survived by: his wife Susan C. Kubiak (Waskiewicz); his daughters, Justine M. Kubiak of Pittsburgh, Julie A. Kubiak (Craig Bixler) of Manassas, VA and Marlene E. Ebling (Alan) of Pittsburgh; and his grandchildren, Carter and Caitlyn Ebling. He is also survived by many nieces and nephews.
