Kozak; his children, Kim Gouker (Pat), Kris Gates (Adam) and David Kozak (Stacey); his grandchildren, Patrick Gouker and Eva Gouker; his stepgrandsons, Aidan Hickey and Liam Hickey; and his cousins, Diane Poff (Clubert) and Christopher Poff.
